55-EDTW VERIFICATION FORM WITH REQUIRED DOCS

EDTW is a tuition waiver program funded by New York State in conjunction with participating educational institutions throughout the State. This program provides tuition waivers to Medical, Dental, Pharmacy and Law students who are determined to be of exceptional need by the Financial Aid Office. This program can provide an award up to the cost of New York State tuition minus other tuition assistance offsets.

EDTW VERIFICATION REQUIREMENTS:

Your application was selected for review in a process called “Verification.”  You have been granted an estimated EDTW award based upon your 2008-09 FAFSA information. For this to become an actual EDTW award, we must compare your verification information with your FAFSA information. Therefore, you must comply with the following requirements:

1. Complete the 2008-2009 EDTW Verification Form
2. Attach signed copies of your (and your spouse’s) and your parent(s)’ 2007 Federal Income Tax Forms (1040, 1040A, 1040EZ)
3. Include all supporting Schedules and W-2 Forms

You will receive a confirmation letter regarding the EDTW verification results. If the reported FAFSA information corresponds with the financial documentation provided, the estimated EDTW award will become an actual award and applied to your student account.
